VHAN Launches Pilot of a Deviceless Remote Patient Monitoring Program 

VHAN is evolving the network’s clinical services and patient engagement strategies to better meet patients where they are. These services include new diagnosis support programs such as Deviceless Remote Patient Monitoring powered by CareSignal, a company owned by Lightbeam Health Solutions.  

This spring the VHAN Care Management team initiated a CareSignal pilot program with a small group of adult patients diagnosed with diabetes and pre-diabetes. Once patients are enrolled in the program, CareSignal engages with them through text-based chats or automated phone calls based on patient preference. The Care Management team monitors responses to provide care interventions and communicate with physicians as needed.  

This program will enable VHAN to support a larger number of patients and allow teams to focus on those with the greatest needs.  More information to come as the program expands.
