VHAN Celebrates Successes & Looks Forward

As VHAN’s executive director and chief medical officer, I want to extend a sincere thank you for your unwavering dedication to ensure patients receive high-quality, personalized care.

As I reflect on our most recent performance year’s successes, I am reminded that our network’s collective strength and commitment enables better health outcomes, including consistently achieving 100% threshold on key quality measures. Since 2012, you’ve generated a remarkable $204 million in shared savings bonuses for health plans, resulting in $77 million directly returned to you in shared savings and quality bonuses.

In my seven years with VHAN, I have been inspired by how our members come together to overcome challenges to serve their communities well. One example is Brownsville Medical Clinic, who after the loss of beloved founder and main provider Jerald White, MD, was supported by a group of hometown heroes who came out of retirement to lend medical and administrative support. Their selfless act kept the clinic open, a relief for residents of their small town who greatly depend on the practice. I encourage you to read more about their story here.

As our clinically integrated network continues to mature, I’m excited to share some innovative programs to support you and your teams in ways that are most meaningful:

  • We’re expanding our population health insights capabilities with OnePoint. This new tool optimizes patient care through information sharing, allowing easy identification of patient needs and decreased administrative burden through efficient workflows.
  • We’re boosting our mental health resources, including adult and pediatric consult lines and a multi-part miniVHAN podcast series on behavioral health through the lifespan. This fall we’ll offer our third Project ECHO (Extension of Community Healthcare Outcomes) program focused on pediatric depression and anxiety. Plus, we offer a wealth of mental health tools on VHAN Hub.
  • Our dedicated VHAN Care Management teamcontinues to showcase success stories in supporting patients with complex health needs. Our nurses, social workers and pharmacists serve as resource extenders for your team to ensure patients receive coordinated care between visits and across care settings.
  • We’re offering new learning opportunities, such as patient-facing Well Moment webinars. Upcoming webinars will focus on parenting for defiant behaviors and suicide prevention. Next month, we’re launching on-demand learning content for coding and risk adjustment.
