VHAN is excited to announce that the network’s pediatric depression screening rate has dramatically improved. In 2019, VHAN began measuring annual depression screening for children 12 and older. To assist members who were struggling in this area, VHAN licensed clinical social workers met with practices to promote the social work program and the benefits of using the pediatric behavioral health consult line. As a result, by 2021, no VHAN members received a score of zero for depression screening and 46% of practices screened 90% of their VHAN patients for depression.

During a recent Pediatrics Champion Call, Kylie Cormier, MD, from Old Harding Pediatrics Associates (OHPA), shared best practices for capturing a yearly chlamydia screening from eligible patients. This important screening is part of the pediatric modifier for 2022, and it is also a required quality measure for many payors. Dr. Cormier and her colleagues have streamlined the screening process, while ensuring that patients and parents feel as comfortable as possible.

In 2020, VHAN focused on 11 key aims to drive quality performance and improve patient care. Our new Impact Report outlines how the network performed across these key aims, including several personal stories that detail how VHAN providers have made a difference in patients’ lives.

The 2020 MSSP ACO program results are in, and VHAN is excited to report that our Connected Care ACOs had a remarkable year.

When VHAN patients obtain care outside the network, the opportunity to impact the cost and quality of that work is diminished if not eliminated. A key priority for the network is access, and in 2020 VHAN met or exceeded in-network access and utilization goals.

Working directly with our member hospital systems and practices, VHAN aims to improve the transition process and improve continuity of care, thus reducing adverse events and readmissions. 

In 2020, the VHAN Pharmacy Team worked with about 1,400 patients to optimize medications for chronic disease management, transitions of care and polypharmacy. Here’s one example of how VHAN pharmacy support helped one patient manage her complex medical conditions.

Care Navigators identify issues that might put patients at risk, such as difficulty getting medications or accessibility issues. Here’s a story of how a Social Work Care Navigator helped mitigate a transportation challenge for a patient.
