Vanderbilt Health Affiliated Network (VHAN)’s Care Management program brings together a team of registered nurses, diabetes educators, social workers and pharmacists to help patients achieve their health goals. If you’re a patient recovering from a recent hospitalization, trying to better manage a chronic disease or navigating a complex medical condition, VHAN’s team of care navigators can provide personalized care for your needs. Answers to health concerns or medication questions
Connection to a preferred healthcare provider
Development of a personalized plan of care
Encouragement to reach health goals
Help to understand follow-up care after an ER visit or hospitalization
Access to community resources, such as social support and grief counseling
